The Affordable Care Act (ACA) transforms the U.S.'s public and private health care financing systems into vehicles for promoting public health by making evidence-based preventive services available nationwide through individual and group health plans, Medicare, and Medicaid. The ACA accomplishes this transformation by breaking down two barriers: (1) the public health-health care divide, which led to a dominance of curative medicine over preventive health measures and (2) ERISA preemption, which created an obstacle to the provision of a uniform set of (...) evidence-based preventive services that could be made available to the U.S. population through individual and group health plans. As a result, prevention measures with proven effectiveness will now be provided on a national and uniform basis to a majority of Americans, with the potential to improve health outcomes and reduce costs. The most prominent — and certainly the most controversial — feature of the Patient Protection and Affordable Care Act is the so-called “individual mandate,” which attempts to address the problem of 50 million uninsured by requiring nearly all Americans, beginning in 2014, to obtain health insurance. While expanded access to health insurance has been both the cornerstone and the lightening rod of the ACA, the Act also contains significant public health provisions focusing on, among other things, promoting the availability of (...) prevention and wellness services. Although these public health provisions have been greeted with mixed reviews, there has been very little discussion of what may be the ACA’s most significant public health feature: the preventive services mandate. In a bold stroke, the ACA changes the way evidence-based preventive services will be provided and paid for by private health insurance plans, Medicare, and Medicaid. By requiring these health plans to provide evidence-based preventive services with no out-of-pocket costs, the ACA transforms the U.S.’s public and private health care financing systems into vehicles for promoting public health. Few figures of the Middle Ages command the attention of so many modern disciplines as Robert Grosseteste (c. 1170-1253). Theology, Philosophy, History, and Science are all areas which his life and thought continue to have significance and to inspire re-interpretation. Accompanied by a series of original commentaries, this new edition of Grosseteste's work, with English translation, draws together the perspectives of modern scientists and medieval specialists. Volume I of a six volume series, Knowing and Speaking presents two of the earliest (...) of Grosseteste's works, the treatises On the Liberal Arts and On the Generation of Sounds. These are accompanied by a significant Middle English treatise on the Seven Liberal Arts whose anonymous fifteenth-century author translated and excerpted passages from Grosseteste's treatises in a re-imagining of their structure and function.0Each work is treated separately within the volume, which is constructed in three parts. Summary Knowledge is an important but largely neglected variable in sex education research. This study aimed to develop a measure to assess young people's knowledge about puberty and sexual development, and to examine knowledge in relation to age, gender and school. The main results of the study were that knowledge increased more between age 11/12 and 13/14 than between 13/14 and 15/16, girls knew more than boys at every age, and there were few differences in knowledge between the four schools (...) involved in the study. The research has a number of implications for sex education in schools. First, it was found that even by age 15?16 some young people lack information which is essential if they are to avoid unwanted pregnancy. Second, it may be that boys and girls need to be taught separately to enable the different needs of each gender to be addressed. Third, young people know more about some aspects of puberty and sexual development than others, and there are particular times when knowledge develops most rapidly. Giving teachers this information could help them to target areas of particular ignorance. The questionnaire developed to assess knowledge proved to be accurate and reliable, and a measure that is straightforward to score and analyse. As such, it has considerable potential for use in the classroom. (shrink)

When the benefits of surgery do not outweigh the harms or where they do not clearly do so, surgical interventions become morally contested. Cutting to the Core examines a number of such surgeries, including infant male circumcision and cutting the genitals of female children, the separation of conjoined twins, surgical sex assignment of intersex children and the surgical re-assignment of transsexuals, limb and face transplantation, cosmetic surgery, and placebo surgery.

The logos has· been interpreted in any way by different scholars and· the · proper ·interpretation seems not to be clear. There seems to be a contradiction in the prologue, Jn 1 :1. Again, what is the relationship of the logos to the father as it appears in the prologue? The purpose of this study is to try an attempt a synthesis of those varied interpretations. The significance of the study is that it will help students of scriptures to have (...) more insight into the understanding of the logos and the prologue. The scope of this study is the passage of John 1: 1-18 and other related material. Scholars and professional organizations in bioethics describe various approaches to “quality assessment” in clinical ethics. Although much of this work represents significant contributions to the literature, it is not clear that there is a robust and shared understanding of what constitutes “quality” in clinical ethics, what activities should be measured when tracking clinical ethics work, and what metrics should be used when measuring those activities. Further, even the most robust quality assessment efforts to date are idiosyncratic, in that they represent (...) evaluation of single activities or domains of clinical ethics activities, or a range of activities at a single hospital or healthcare system. Countering this trend, in this article we propose a framework for moving beyond our current ways of understanding clinical ethics quality, toward comprehensive quality assessment. We first describe a way to conceptualize quality assessment as a process of measuring disparate, isolated work activities; then, we describe quality assessment in terms of tracking interconnected work activities holistically, across different levels of assessment. We conclude by inviting future efforts in quality improvement to adopt a comprehensive approach to quality assessment into their improvement practices, and offer recommendations for how the field might move in this direction. Arthur C. Danto's essays not only critique bodies of work but reflect upon art's conceptual evolution as well, drawing for the reader a kind of "philosophical map" indicating how art and the criteria for judging it has changed over the twentieth century. In _Unnatural Wonders_ the renowned critic finds himself at a point when contemporary art has become wholly pluralistic, even chaotic-with one medium as good as another-and when the moment for the "next thing" has already passed. So the theorist (...) goes in search of contemporary art's most exhilarating achievements, work that bridges the gap between art and life, which, he argues, is now the definitive art of our time. Danto considers the work of such young artists as John Currin and Renee Cox and older living masters including Gerhard Richter and Sol LeWitt. He discusses artists of the New York School, like Philip Guston and Joan Mitchell, and international talents, such as the South African William Kentridge. Danto conducts a frank analysis of Matthew Barney's _The Cremaster Cycle_, Damien Hirst's skeletons and anatomical models, and Barbara Kruger's tchotchke-ready slogans; finds the ghost of Henry James in the work of Barnett Newman; and muses on recent Whitney Biennials and art influenced by 9/11. He argues that aesthetic considerations no longer play a central role in the experience and critique of art. Too much good philosophy on too many topics has been done recently—much of it using technical devices—to allow him or anyone else, to master more than a few parts. That seems right to me.The first volume consists of five parts: One: G. E. Moore on Ethics, Epistemology, and Philosophical Analysis; Two: Bertrand Russell on Logical and Linguistic Analysis; Three: Ludwig Wittgenstein's Tractatus; and Four: Logical Positivism, Emotivism, and Ethics; and Five: The Post-Positivist Perspective of the Early W. V. Quine. The great figure associated with twentieth-century analytic philosophy who is missing from Volume 1, Gottlob Frege, Soames omits, because Frege's "concerns were, on the whole, more specialized and technical than the others, and for many years this limited his influence" (1:xiii n. 1). Soames thinks its "two most important achievements" are (i) the recognition that philosophical speculation must be grounded in pre-philosophical thought, and (ii) the success achieved in understanding, and separating one from another, the fundamental methodological notions of logical consequence, logical truth, necessary truth, and apriori truth" (1:xi; see also 1:298-99 and 2:252). He admirably succeeds in making the case for these judgments, although many philosophers will think that these two together do not amount to a great deal, considering all the effort expended by analytic philosophers. I think that analytic philosophy has done more; it has given philosophers many techniques for attacking conceptual problems, a great deal of information about the structure and nature of language, and has decisively refuted various theses that were once thought attractive and even obviously true. In any case, those who think that analytic philosophy, especially conceptual analysis, accomplished nothing are, I think, refuted by these volumes. In his discussion of G. E. Moore's article, "A Defense of Common Sense," Soames explains what he takes to be the first great result of analytic philosophy. Moore showed that the beliefs of common sense should often be accepted "with more confidence" than any philosophical beliefs or propositions that conflict with them, and in these cases cannot be overthrown by philosophical beliefs. For example, "The skeptic assumes that we can be certain about what knowledge is before we decide whether what we all ordinarily take to be paradigmatic cases of knowledge really are genuine" [End Page 125] (23, entire sentence is bold in the original). (Soames uses boldface type to mark all of the significant propositions discussed in the book, a nice pedagogical device.)In his discussion of Bertrand Russell's theory of descriptions, Soames gives the impression that linguistic interests primarily motivated Russell. I think the motivation was primarily ontological. Russell wanted to justify ridding philosophy of nonexistent entities. The doctrines of Wittgenstein's Tractatus Logico-Philosophicus and Logical Positivism were two ways in which philosophical analysis developed. For Soames, what is most important in the Tractatus is the theory of names, and he holds, I believe, the minority belief (which I share) that properties are reducible to configurations of objects (1:206-6). What is most important in Logical Positivism is the verification theory of meaning. More scholars are producing excellent works in this area than ever before. A large part of this health is due to scholars whose primary training is not in philosophy, such as historians of (...) political thought or literature (hereafter: historians). The dictionary being reviewed here is a case in point, a production of more than ninety scholars of whom the overwhelming majority are not formally trained philosophers.The dictionary consists of about 400 entries, alphabetically from Isaac Abendana (d. 1699) to Richard Zouche (1590-1661) and chronologically from John Case and Richard Hooker, both of whom died in 1600, to Richard Mead (1673-1754). As this list indicates, the general editor construed "philosopher" very broadly, too broadly, I think. There should be no objection to including scientists like Robert Boyle and Newton, theologians like William Chillingsworth and Henry Hammond, and poets like Andrew Marvell and John Milton, each of whom wrote some works with philosophical import. But Ralph Bathurst, a physician, comes no closer to being a philosopher than being "an early admirer of Hobbes's scientific work and [having] contributed some enthusiastic Latin verses to the latter's Humane Nature (1650)" (75a). Likewise Richard Barckley (fl. 1598), about whose life virtually "[n]othing is known" and whose sole book may be described as consisting of "meatpies stuffed with commonplaces"—Montaigne's phrase—does not deserve inclusion (55a, 56a). (Jill Kraye, the author of the article, serves Barckley very well.)Ramist logicians "are largely excluded... [because] they tended to assimilate logic to rhetoric and to limit it to the orderly exposition of what is already known" (p. x). Nonetheless, it is unfortunate, I think, that Abraham Fraunce (c. 1558-1633) and George Downham (d. 1634) are not included.The best articles tend to be about the best philosophers of the age: Thomas Hobbes and John Locke by G. A. J. Rogers, Shaftesbury by David McNaughton, and Francis Bacon by Graham Rees (who pointedly observed about Historia ventorum that "Bacon accomplished the considerable feat of making the weather boring" [41b]). Although by quality of thought and influence, each of these warrant longer treatment, the editor was right to restrict their length to a modest six or seven pages, the same length as those about William Ames, William Harvey, Bernard Mandeville, and John Toland. There are plenty of other places to go for information about the great philosophers. What we want and what the Dictionary provides are articles about the obscure philosophers.Some of the other especially well wrought articles are those on Richard Baxter by William Lamont, Walter Charleton by Margaret J. Osler, Joseph Glanvill by Richard Popkin, John Hales by Victor Nuovo, John Wilkins by John Henry, and Thomas White by Beverley Southgate.The editor is not nationalistic in his understanding of "British." Scots who spent their academic career on the Continent are included. Also included are the Czech John Comenius (1592-1670), the Dutchman Franciscus Mercurius van [End Page 598] Helmont (1614-98), and the Venetian Marc Antonio De Dominis (1560-1624), as they should, because of their influence on British thought or sometime residence. By the same token, Hugo Grotius should have an entry; but he does not.An average of two pages each is devoted to six women, Mary Astell, Catherine Cockburn, Ann Conway, Margaret Cavendish, Jane Lead, and Damaris Cudworth Masham. They deserve more space, especially Cavendish, whose The World's Olio (1655) includes a utopian scheme.Some other articles that I wish had been longer include those about John Bramhall, Richard Crackanthorpe, William Davenant, and John Eacherd, each roughly two pages. Although the proportional space allotted to a thinker is generally right, I think some figures should not have been included at all. John Flavel (1596-1617) does not deserve even the 3 column inches devoted to him. Better to have assigned those inches to someone else. Unfortunately, not far into reading Aspects of Reason, Wittgenstein's quip, "A philosophical problem has the form: 'I don't know my way about'" (Philosophical Investigations, paragraph 123) came to mind and it did not leave me for the rest of the book. (See Grice's own admission on page 4.)As best I can tell, Grice has two main goals. The first is to show that theoretical (or alethic) reasoning and practical reasoning are at bottom the same. Aristotle and Kant held the same thing, as Grice notes more than once. Pace many philosophers who hold that 'ought' does not mean the same thing in the sentences 'She ought to repay the loan' and 'He ought to be home by this time,' Grice holds that it does. That is, the practical 'ought' and the alethic 'ought' have the same sense or senses. While the thesis is clear enough, Grice's path to proving it is not sufficiently clear. He claims that the same general sense of 'rational acceptability' is involved in each sentence. This claim is not informative because the idea of acceptability is no clearer than the claim roughly that 'ought' has the same sense in both its alethic and practical uses. (The editor Richard Warner recognizes this, and tries to shore up Grice's view; see xvi-xvii.)The second of Grice's main goals is to prove that modals in both alethic and practical sentences are conditional. In effect, sentences that say something ought to be the case or ought to be done are conditioned on some premises from which the sentence is or may be logically derived. (Nonetheless, he thinks some sentences with modals are a priori and underived.)There are too many promissory and subjunctive gestures like this: "I suspect that such a reply could be constructed, but I do not have it at my fingertips (or tongue-tip), so I shall not try to produce it." And too many prepositional phrases, under the shared command of abstract nouns and verbs in the passive voice, march single file through the pages, e.g.: "a brief consideration of what modifications might be required or convenient for the employment of mode-markers in the representation of the content of thought (or acceptance)" (70). Some of the few substantive results seem to me to be prolix: Let us suppose it to be a fundamental psychological law that, ceteris paribus, for a creature x (of a sufficiently developed kind), no matter what A and B are, if x wills A and judges that if A, A only as a result of B, then x wills B. This I take to be a proper representation of "he who wills the end, wills the indispensable means"; and in calling it a fundamental law I mean that it is the law, or one of the laws, from which 'willing'... John Dewey gives an account of meaning that stands in significant contrast to contemporary theories of meaning. A readily apparent difference is Dewey's subordination of truth to meaning while much recent philosophizing about meaning, particularly in what is often referred to as the Analytic tradition, subordinates meaning to truth. Another difference, and one that helps account for the first, is philosophic method: Dewey is explicitly empirical in his attempt to understand meaning while prominent thinkers in the Analytic tradition have (...) come to reject empiricism as if it could mean only the empiricism of Locke and Hume . ;To understand these general differences in more detail, the present work contrasts Dewey's account with a particularly well-known theory of meaning in the Analytic tradition, that of Donald Davidson. Davidson proposes a theory of meaning that is strongly influenced by a rejection of an empirical approach. This non-empirical theory of meaning strives to account for the possibility of certainty in understanding, but it does so by placing serious limitations on meaning. Dewey's empirical approach removes these limitations by replacing the desire for absolute certainty with faithfulness to experience in inquiry. ;The main aim of the present work is to give a clear explication of Dewey's empirical approach to meaning and indicate its advantages for philosophy and human life. The present work follows Dewey in tracing the development of meaning from its experiential roots to its continued growth in interaction. Dewey's approach results in an evolutionary or developmental account of meaning. His account makes sense of extra-linguistic meaning, kinds of meanings, and new meanings that grow out of novel experiences. ;On Dewey's account philosophical activity shifts emphasis from truth to meaning. Truth is not denigrated or denied, rather it is now understood in the broader context of experience. Truth as a metaphysical concept is disambiguated from truths or warranted assertions as the product of specific inquiries; and the latter serve meanings as means to experiences, which includes more than cognitive experiences.
